About the Dissociation Pattern Tracker

This clinician worksheet provides a structured way to observe and document dissociative responses that may emerge during therapy sessions or between sessions. It allows therapists to record patterns such as spacing out, memory gaps, emotional numbing, sudden shifts in affect, or changes in present-moment awareness.
